P0401 on a 2014-2018 Ram 1500 3.0L EcoDiesel means the PCM asked for EGR flow and measured less than it requested for 15 seconds. Work the checks in order: software on a 2017 truck, air flow sensor codes, air filter, leaks, then the EGR parts. EGR and diesel particulate filter regeneration are off while it is active; coolant loss you cannot explain, or no heater, means check the VIN for the EGR cooler recall.

Safety first

  • Wear safety glasses when blowing low pressure compressed air through the EGR cooler.
  • Let the cooling system cool before opening it; hot coolant under pressure causes serious burns.
  • Antifreeze is ethylene glycol, harmful if swallowed or inhaled; dispose of used coolant properly.
  • Disconnect and isolate the negative battery cable before removing the EGR cooler or EGR valve.

What does P0401 mean on a 2014-2018 Ram 1500?

P0401 (EGR Flow Insufficient Detected) is the flow check on the EGR air flow control valve loop of the 3.0L diesel.

The valve is a pulse width modulated output from the PCM, mounted on the intake manifold. It sets the ratio of fresh air to EGR gas and rests open when not powered. The PCM watches that ratio with the mass air flow sensor, whose feedback is a big input to where the PCM puts the valve. So a mass air flow sensor reading wrong can upset EGR control even with no code stored.

P0401 is the standard heat-up monitor for insufficient flow; P04DD is the rapid heat-up monitor for the same fault, and both can be present.

The PCM sets P0401 when EGR flow runs below the requested flow for 15.0 seconds. All of these must hold:

  • The engine is running.
  • The check runs continuously while it is.
  • Measured flow stays below the request for the full 15.0 seconds.

The PCM then turns the check engine light on. While the fault is active, the EGR system and diesel particulate filter regeneration are disabled.

Three sibling codes use the same logic with different thresholds:

  • P0402 (EGR Flow Excessive Detected, standard heat-up) sets on flow above the request for 10.0 seconds, engine running, ambient above 19.4 degrees F (-7 degrees C).
  • P04DD (Cold Start EGR 1 Flow Insufficient Detected, rapid heat-up) sets on flow below the request for 15.0 seconds, engine running.
  • P04DE (Cold Start EGR 1 Flow Excessive Detected, rapid heat-up) sets on flow above the request for 10.0 seconds, engine running, ambient above 19.4 degrees F (-7 degrees C).

What symptoms does P0401 cause on the Ram 1500?

  • The check engine light comes on.
  • The EGR system and diesel particulate filter regeneration are disabled while the fault is active.
  • You may see a Service Electronic Throttle Control message, and a turbo underboost code can be stored alongside P0401.

What causes P0401 on the Ram 1500, most likely first?

These are the causes in the order I would work them, with the quick check and the usual fix for each.

The maker issued Technical Service Bulletin 18-068-20 for the 2017 truck; it is a PCM software update that lists P0401-00 with the check engine light on, which is the same item as cause number 1. Separately, Safety Recall VB1 (NHTSA 19V-757) covers the EGR cooler, which can crack from thermal fatigue, and it is a check by VIN rather than a cause of P0401.

How do you diagnose P0401 on a 2014-2018 Ram 1500?

You will need a scan tool, a battery charger, a small inspection mirror, soapy water, an intake pressure tester, low pressure compressed air and safety glasses.

  1. 1

Read codes in every module and save the scan report.

Check the repair history, check every module for available software and read the bulletins for the truck. Erase all codes, leave the key off at least 10 seconds, key on, then run or drive the truck under the conditions that set the code and read codes again. That tells you whether the fault is live and whether a software update applies first. If it does not return, treat it as an intermittent and do not replace parts on it.

    • On a 2017 truck, reprogram the PCM only if its software is not the latest, and put the TCM on its latest software too. If the PCM is already current, the bulletin is complete.
    • Keep a charger on the truck during the flash, restart the flash if it is interrupted, and clear any codes other modules set during it.
  2. 2

If P0101, P0102 or P0103 is active or pending, work that code's own diagnosis before going further. The mass air flow sensor is the PCM's yardstick for EGR flow, so bad data from it can look like low EGR flow. If none are present, carry on.

  3. 3

Key off, pull the air filter and look for dirt or debris that would restrict air; replace the element if you find any.

If the filter is fine, check the fresh air inlet tube for a blockage from snow, ice or dirt, and look over the intake, crankcase vent and turbocharger tubes, plus the exhaust system and its connections, for damage, restriction or a poor connection. Repair what you find.

  4. 4

Pressure test the intake air system (charge air cooler and hoses, intake manifold and the EGR system) and check for leaks with soapy water.

Repair any leak. Then look over the EGR system for soot streaks around the valve and connections and repair any leak you find.

  5. 5

Disconnect the air tube at the inlet port of the EGR air flow control valve, put a mirror on the valve and turn the ignition on.

It should open completely without binding. Cycle the ignition on and off several times while you watch. If it does not move freely, replace the valve.

  6. 6

Key off and get to the EGR cooler and both tubes.

Check for damage, leaks, heavy soot and anything restricting flow. Put low pressure compressed air at the cooler's lower port; it should flow out the upper port. If you find a problem, clean or replace the cooler or tubes as needed. If you find none, replace the EGR valve, which means taking the cooler off first. On refit, use new gaskets and O-ring seals and check that the EGR cooler vacuum bypass hose is fully seated with no gap.

What if the obvious fix does not work?

Go back to the mass air flow sensor; it can feed the PCM inaccurate readings even with no code stored.

The classic mistake is replacing the EGR valve before inspecting the cooler and tubes; the valve only gets replaced when they check clean. Another is reusing old gaskets or O-ring seals; they all go back new.

Coolant loss you cannot explain or no heater output could point to a cracked EGR cooler, so have the VIN checked for the recall.

Here is the stop point. If the code returns after the repair, the repair is not complete. Check the related bulletins and go back to the code list. A guide cannot diagnose your truck from a distance.

How do you confirm the repair?

The road test below is required after the repair.

  1. Confirm all engine components are installed and connected.
  2. Erase all codes with the scan tool.
  3. Road test the truck under varied driving, and try to repeat the freeze frame conditions where you can.
  4. At some point, stop, turn the engine off for at least 10 seconds, restart it and keep driving.
  5. At the end of the drive, read codes.

No code means the repair worked; if the code is back, the repair is not complete.

Two groups of related codes matter here. P04DD is the rapid heat-up twin and uses the same diagnosis. The mass air flow sensor family, P0101 (performance), P0102 (circuit low) and P0103 (circuit high), gets fixed first if any of them is active or pending.

Questions mechanics ask about P0401 on the Ram 1500

What does P0401 mean on a 2014-2018 Ram 1500 EcoDiesel?+

The PCM asked for EGR flow, measured less than it requested for 15.0 seconds with the engine running, and turned the check engine light on. On this truck that points you at the air filter, intake leaks or restrictions, the EGR air flow control valve, EGR leaks, a restricted EGR cooler and tubes, and the EGR valve.

Can you keep driving a Ram 1500 EcoDiesel with P0401?+

Plan on fixing it, because the EGR system and diesel particulate filter regeneration are both disabled while the fault is active. Coolant loss you cannot explain or no heater output are warning signs of a cracked EGR cooler. If you have either one, have a Ram dealer check the VIN for the recall.

Is there a Technical Service Bulletin for P0401 on the Ram 1500?+

For the 2017 truck, Technical Service Bulletin 18-068-20 lists P0401-00 with the check engine light on. The fix is a PCM reprogram only if the software is not already the latest, plus the TCM on its latest software. If the PCM software is already current, the bulletin is complete.

Is there an EGR cooler recall on the 2014-2018 Ram 1500 EcoDiesel?+

Yes. Safety Recall VB1 (NHTSA 19V-757) covers the EGR cooler, which may crack from thermal fatigue. The remedy is a new EGR cooler assembly, done at no charge to the owner. With the check engine light on and P0299 or P2D2F present, the intake manifold is also inspected and pressure tested, and replaced if perforated. Have a dealer run your VIN.

What else sets with P0401 on the Ram 1500?+

P04DD is the rapid heat-up version of the same fault and can appear alongside P0401. The mass air flow sensor codes P0101, P0102 and P0103 get worked first. A turbo underboost code can also be stored alongside P0401.
